Terms & Conditions

Please read the following agreement carefully before registering for any program at Uinta Soccer Academy.

Legal Disclaimer

1. Definitions

"Participant" refers to the minor child listed above who will take part in soccer training activities.

"Parent/Guardian" refers to the individual legally authorized to sign this Agreement on behalf of the Participant.

"Academy" refers to Uinta Soccer Academy, LLC, including its owners, coaches, staff, volunteers, contractors, and representatives.

"Activities" refers to all soccer training, conditioning, drills, scrimmages, events, and related participation, whether on or off the field.

2. Assumption of Risk

The Parent/Guardian acknowledges and agrees that:

  • Soccer is a physical, contact sport that involves inherent risks, including but not limited to: sprains, strains, fractures, concussions, head injuries, collisions, falls, impacts with equipment or other participants, weather‑related injuries, and other unforeseen hazards.
  • These risks cannot be eliminated without jeopardizing the essential nature of the activities.
  • The Parent/Guardian voluntarily chooses to allow the Participant to engage in the Activities with full understanding of these risks.
  • The Parent/Guardian accepts and assumes all risks of injury, illness, or harm, whether known or unknown, foreseeable or unforeseeable.

3. Release of Liability and Waiver of Claims

To the fullest extent permitted by Utah law, the Parent/Guardian hereby:

  • Releases, waives, and discharges the Academy from any and all claims, demands, causes of action, damages, or liabilities arising out of the Participant's involvement in the Activities.
  • This release includes claims for ordinary negligence by the Academy or its staff.
  • The Parent/Guardian agrees not to sue the Academy for any injury, illness, or harm sustained by the Participant during Activities.
  • The Parent/Guardian understands that this waiver does not apply to claims arising from gross negligence, willful misconduct, or conduct that cannot legally be waived under Utah law.

4. Payment & Refund Policy

The Parent/Guardian understands and agrees that:

  • Monthly training fees must be paid either in full at the time of enrollment or according to the Academy's approved split‑payment schedule selected during registration.
  • Optional Gear Fee: A one‑time $40 gear fee is available for families who choose to purchase an Academy training top. This gear purchase is optional and not required for participation. Additional gear may be purchased through our partner's storefront.
  • No Mid‑Month Refunds: Once the monthly training period has begun, all fees are non‑refundable. The Academy does not issue partial refunds, prorated credits, or adjustments for missed sessions, voluntary withdrawal, schedule conflicts, injury, illness, dismissal due to conduct violations, or any other reason occurring after the start of the month.
  • Late Payment Policy: Payments not received by the stated due date are considered late. A late fee of $10 may be applied to any outstanding balance. The Academy reserves the right to suspend the Participant from training sessions until the account is brought current. Continued non‑payment may result in removal from the program without refund.
  • Payment Schedule & Unused Sessions: The Academy does not adjust or postpone upcoming payment dates when a Participant reschedules a session to a later week. If a Participant has unused sessions at the time their next payment is processed, those unused sessions will be added to the total number of sessions included in the new package. All payments will continue to follow the original billing schedule selected at enrollment.
  • Subscription & Auto‑Renewal Policy: By registering for any training package, the Client acknowledges and agrees that their enrollment operates on a recurring subscription that will automatically renew according to the billing schedule selected at registration. For monthly plans, the next payment is processed one month after the initial payment. For biweekly plans, the next payment is processed every two weeks after the initial payment. Subscriptions continue to renew automatically until the Client submits a valid cancellation request in accordance with the Academy's Auto‑Renewal Cancellation Notice policy.
  • Auto‑Renewal Cancellation Notice: Clients wishing to cancel or pause their auto‑renewal must submit a written request via email to uintasocceracademy@gmail.com at least three (3) days prior to their upcoming billing date. Requests submitted with less than three days' notice may not be processed in time, and the scheduled payment may still be charged.
  • Failed Payment & Grace Period Policy: If a scheduled payment fails to process for any reason, a seven (7)‑day grace period will begin on the date of the failed payment. During this grace period, the Academy will attempt to contact the Client to address the failed payment and provide an opportunity to update or correct the payment method. If the Client does not resolve the failed payment within the seven‑day grace period, the Client's subscription will be cancelled. To resume training, the Client will be required to register again under the current rates and policies.

5. Indemnification

The Parent/Guardian agrees to indemnify and hold harmless the Academy from any claims brought by or on behalf of the Participant, or by third parties, arising from the Participant's actions during Activities.

6. Medical Authorization

The Parent/Guardian:

  • Authorizes the Academy to obtain emergency medical care for the Participant if the Parent/Guardian cannot be reached.
  • Accepts full financial responsibility for any medical treatment or transportation.
  • Confirms that the Participant is physically able to participate and has no medical conditions that would make participation unsafe.

7. Concussion and Head‑Injury Acknowledgment

The Parent/Guardian acknowledges:

  • Receipt of information regarding concussion risks and symptoms.
  • Understanding of Utah's Return‑to‑Play requirements.
  • Agreement that the Participant will be removed from training if a concussion is suspected and will not return until cleared by a qualified health professional.

8. Weather, Field, and Equipment Disclaimer

The Parent/Guardian understands and accepts that:

  • Outdoor fields may have uneven surfaces, holes, debris, or variable conditions.
  • Weather conditions (heat, cold, wind, rain, snow, lightning) may change rapidly.
  • Equipment used during training may vary in condition and may pose risks inherent to the sport.

9. Communicable Disease Acknowledgment (Including COVID‑19)

The Parent/Guardian acknowledges:

  • Participation may expose the Participant to viruses, bacteria, and communicable diseases.
  • The Academy cannot guarantee a disease‑free environment.
  • If the Participant is showing symptoms of a communicable disease, it is the Parent/Guardian's responsibility to disclose that information to the Academy. The Academy may exclude the Participant from training sessions, and no refunds or credits will be issued for missed sessions.
  • The Parent/Guardian agrees to follow all health protocols implemented by the Academy.

10. Photo and Video Release (Optional Opt‑Out)

The Parent/Guardian:

  • Grants permission for the Academy to photograph or record the Participant during Activities.
  • Allows the Academy to use such images for marketing, social media, and promotional purposes.

Opt‑Out Option:

If you do NOT consent, please notify the Academy in writing at registration so the Participant is not included in promotional media.

11. Code of Conduct and Dismissal Policy

The Parent/Guardian acknowledges that:

  • Participants must follow all Academy rules, instructions, and behavior expectations.
  • Unsafe, disrespectful, or disruptive behavior may result in removal from the program without refund.
  • The Academy reserves the right to dismiss any Participant whose behavior jeopardizes safety or program integrity.

12. Missed Session Policy

Sessions may be rescheduled and rolled over to another available training day only when the Academy is notified at least 24 hours in advance of the Participant's inability to attend their scheduled session. If the Academy does not receive at least 24 hours' notice and the Participant does not attend, the missed session will be counted as used and will not be eligible for rollover, credit, or transfer.
